Housing instability, unemployment, legal challenges, healthcare access, and family circumstances all shape a person's capacity to recover and remain well. SUD Case Management at The Garage exists to navigate those realities alongside you connecting you to the community resources, behavioral health services, and practical support systems that make sustained recovery possible.
Each client assigned a case manager at The Garage begins with a comprehensive needs assessment. This process identifies not just what services a client needs today, but what barriers exist that could interrupt their recovery tomorrow. From there, the case manager works collaboratively with the client to build a coordinated service plan identifying referral partners, scheduling appointments, following up on outcomes, and adjusting the plan as circumstances evolve.
Case managers at The Garage act as advocates. If a client is struggling to access Medicaid benefits, find stable housing, enroll in a job training program, or navigate the court system as part of a diversion program, the case manager is in their corner removing barriers and creating continuity of care across multiple providers and systems. This service is especially critical for clients who have historically fallen through the cracks of disconnected service delivery.

SUD Case Management is designed for individuals whose recovery is affected by multiple, intersecting needs including those who are newly released from incarceration, experiencing housing instability, navigating the child welfare system, managing co-occurring medical conditions, or who have had difficulty sustaining engagement with treatment in the past. It is also appropriate for clients who need help coordinating care across multiple providers.
